Transcript Request

NCHS alumni may request to have their transcripts mailed to a college or other organization in one of two ways:

1. Stop into the school office and see Julie Snider.  You must pay $5 at the time of the request.  You need to know the name and address of where the transcript is to be sent.
2. Or, fill out the convenient online form below.  Your request will be processed after you have completed a transaction on PayPal (with either your PayPal account or your credit card) for $5.
