Free Sports Physicals offered by LMH
Parents of Students Planning to Attend Grades 9-12 at Newark Catholic High School in the Fall,
Please find attached a flyer regarding the upcoming free sports screening program offered by Licking Memorial Health Systems. Boys are being offered Tuesday, June 2, both from 5:00-6:30 pm. Girls may attend on Monday, June 1 from 5:00-6:3 0pm.
The free sports screenings are being conducted at the Pediatric Office of Licking Memorial Health Professionals located at 1865 Tamarack Road in Newark.
The student packets for the LMHS Pre-participation Sports Screening Program are now posted on the LMHS website. You can access the packets at the link below.
- All students entering the 9th-12th grade for the 2015-2016 school year will need to utilize the PrivIT profile as the OHSAA standard for high school student physical exams this year. They should complete the profile and bring the printed medical history summary, physical examination form, and signed consent to participate the night of the

Please note: Cardiac screenings will be provided to student athletes entering grades 9 and 11. High school students in these grades will be provided EKG screening, in addition to Echocardiogram screening, through mCORE (contracted by LMHS).
During this year’s program, cardiac screening will not be applicable to students entering grades 10 and 12 for the 2015-2016 school year.
As is mentioned above, beginning with the 2015-2016 school year, high school students (grades 9-12) will need to utilize the PrivIt Profile link located at http://www.e-ppe.com. Type “Newark Catholic” in the search box. Instructions for completion are posted on the website. You will need to bring the printed medical history summary, physical examination form, and signed consent to participate with you the night of the free sports screening. Please be sure all forms are completed (including the on-line medical history and forms) prior to your arrival.
